Step 1: Pay first the $8.00 at the Unit 5 online payment link found at the site below. (make
sure link says clinicals) it is ok to set up account as no promotional or marketing
emails will be sent

Step 2:
Tutors, Clinical Experiences, Student Observers, and Field-Based Students not working with an LBS1 program: Please bring a state-issued photo ID for check-in at the building where you are conducting your observation. You must sign in and out each time you visit, so be sure to allow enough time for this process. Entry into the building will not be permitted without signing in. |
Step 2 : Email bwells@iwu.edu for a link to the Unit 5 Background Check site.

| Step 3: Complete Unit 5 criminal background check application via the link received on email and save completion email from Bushue for signing forms - Unit 5 states unless you hear from them everything is good to go. Bring IWU ID to placement. | |

FAQ answers:
- Complete the criminal background check process by the second week of class.
- Allow 2 weeks for district to complete the criminal background check process.
- If placed in a district other than D87 or U5 contact district office for criminal background check procedure.
- IWU students are responsible for fees associated with criminal background checks.
- Districts cannot share criminal background checks.
- Students who do not comply will be removed from placement.
- District requirements are subject to change.
