Forms For Athletes

The following forms are to be printed, completed, and reviewed by the athlete before  bringing them to the Head Athletic Trainer or Assistant Athletic Trainer for approval.  All athletes must have these forms completed before they can begin practice for the athletic season.

Freshman, Transfer Students

Sophomores, Juniors, and Seniors
NEW - Please provide a copy of your insurance card with your forms
Medical Addendum

*** IMPORTANT***
All athletes must have a physical completed by their primary health care provider before their freshmen season and turned in to the Athletic Training Office before they can participate.
